Designed for GPs, practice nurses and practice managers, thishub provides practical information, contacts and support to help streamline the delivery of care for older patients living in aged care homes across Busselton and the Augusta Margaret River region.

We understand that general practice teams are already under significant pressure, often managing increasingly complex care needs within limited consultation time. GPACI has been designed to support providers through improved coordination, clearer communication pathways and stronger collaboration between aged care providers, GPs and primary healthcare teams.

The program aims to improve continuity of care, strengthen GP involvement in aged care, andsupport better health outcomes and experiences for older people living in residential aged care homes.

Capecare is delivering the General Practice in Aged Care Incentive (GPACI) program in partnership with the WA Primary Health Alliance (WAPHA), with dedicated Primary Care Coordinators available to support practices and help facilitate communication and care coordination across participating services.

  • GPACI - GPs

    This questionnaire seeks to understand how GPs are engaging with the General Practice in Aged Care Incentive (GPACI), including awareness, current use, and operational barriers to implementation.

    The aim is to identify practical challenges and support needs within general practice settings to enable more consistent and sustainable participation in aged care delivery.

    Your feedback will help inform future training, system improvements, and support resources for general practices involved in aged care.

    The survey will take approximately 5–10 minutes to complete. All responses will be kept confidential and reported in aggregated form only.

  • GPACI Survey – Practice Managers & GP Owners

    This questionnaire seeks to understand how general practices and GP networks are engaging with the General Practice in Aged Care Incentive (GPACI), including awareness, current use, and operational barriers to implementation.
